Instructions for PSFC Authors: Report Dissemination Form: "The Blue Form"

 

Once a report number has been assigned by the Library, a PSFC Report Dissemination Form (known as the "blue form") must be completed for each report, to ensure accurate tracking for the Library archives and for submission to ILP &  DOE. 

As of October 1, 2008, the "Blue Form" is paperless -- requiring an interactive, online submission.

• IMPORTANT NOTE: You must have been assigned a number by the Library before proceeding.
  
  In order to complete the online form, you need the following basic information:

Report number

Complete title, exactly as it will appear in our database and on the HTML abstract page.

All authors, exactly as they will appear in our database and on the HTML abstract page.

The abstract in TEXT only format.

Keywords, which will be used for indexing. You will be given a selection list and the
option to make your own.

 

Aside from the basic information listed above, the answers you provide to prompts and questions will allow you to continue (or NOT) through the script. For clarifications and further instructions, contact the Library staff.

 

The online form is for JA and RR reports, ONLY. For IR reports, contact the Library staff for special instructions.
